Health Self Assessment Questionnaire

  1. Are you or anyone in your household experiencing any of the following:
    • · severe difficulty breathing (e.g., struggling for each breath, speaking in single words)
    • · severe chest pain
    • · having a very hard time waking up
    • · feeling confused
    • · lost consciousness

    If yes – Call 911 or go directly to your nearest emergency department.

  2. Are you or anyone in your household experiencing any of the following:

    • · shortness of breath at rest
    • · inability to lie down because of difficulty breathing
    • · chronic health conditions that you are having
    • difficulty managing because of your current respiratory illness

    If yes – STAY HOME and call 811 to speak with a nurse.

  3. Do you or anyone in your household have any of the following:

      · fever

    • · new onset of cough or worsening of chronic cough
    • · new or worsening shortness of breath
    • · new or worsening difficulty breathing
    • · sore throat
    • · runny nose

    If yes – STAY HOME. You are legally required to self-isolate.

If you DO NOT get tested – you must isolate for 10 days from the onset of symptoms AND
until symptoms have resolved (whichever is latest). Individuals who are tested, MUST isolate pending their results and should visit bccdc.ca for further information regarding the next steps they must take following their test results.If your symptoms worsen, call 811. Call 911 if you are seriously ill and need immediate medical attention. Inform them that you may have COVID-19.
